In recent years, social and health integration has undergone a significant acceleration towards multidimensional evaluation models, moving beyond sector-based approaches. The Multidimensional Evaluation Unit (UVM), implemented at district level, addresses the need to place the non-self-sufficient person at the centre, promoting an approach that considers not only health factors but also social and environmental dimensions.
The ICF (International Classification of Functioning, Disability and Health) has established itself as a conceptual and operational framework grounded in the biopsychosocial model. This tool analyses functioning and disability as the result of the interaction between health conditions and environmental factors, using hierarchical categories that describe the person in their full complexity. With its neutral language, the ICF facilitates collaboration among social, health and educational professionals, supporting the development of personalised life plans.
The training solutions offered by Synergia aim to disseminate the principles of the ICF and the biopsychosocial model. The contents include an introduction to the principles of the UN Convention on the Rights of Persons with Disabilities, a presentation of the four components of the ICF (body structures, body functions, activities and participation, environmental factors), and an in-depth look at ICF qualifiers and their practical use. Participants will explore the application of the ICF in real settings through practical exercises based on concrete cases and multidimensional assessment tools, with a particular focus on the multiprofessional work of the UVMs.
