One of the main challenges for welfare systems
The integration of social and health policies represents one of the most complex and urgent challenges in contemporary welfare, particularly in the field of long-term care. The aging population and the increase in social-health issues have led to a growing demand for integrated services capable of providing comprehensive care and ensuring effective continuity of care.
In the last decade, the sector has seen significant moments of debate and innovation, such as the introduction of the ICF (International Classification of Functioning, Disability and Health), the widespread adoption of individualized care plans, the establishment of a single access point (PUA), attention to comprehensive care and the “after us” concept, and the new WHODAS assessment tool.
Additional perspectives, such as the employment of people with disabilities, the use of assistive technologies, and overcoming sector-based approaches, are redefining the approach to disability and non-self-sufficiency. The goal is to achieve effective integration not only between social and health policies, but also among institutional bodies (Regions, ASLs, individual and associated Municipalities) and operational services.

Synergia has supported a wide range of local, regional, public, and private entities over time in the development of integrated systems for social-healthcare management and evaluation. Our work has focused on promoting shared languages and the adoption of multidimensional assessment tools, fostering an integrated approach that encourages collaboration among different stakeholders and institutional levels.
With a scientific and operational approach, we have assisted our clients in the experimentation, validation, and adoption of integrated informational systems to support case management, multidimensional assessment, intake processes, as well as the implementation of information flows for monitoring and evaluating services with a high degree of social-health integration (with particular reference to home care and residential care services).
